Since I'm lazy, break this down for me...
My $$$ gets me a 40 hr Tivo box with a lifetime subscription? As in I never have to pay subscription or service fees EVER again?

Yes that is correct.
you have two options when you set up service: monthly atr $12.95/month or Lifetime for $300.
I chose the lifetime route.
I never set it up though cause I dont have a phone line in my apartment and you have to have a phone line to set it up and then after that it can use a phone line or the internet to download the program schedule.
