How does that happen? Timing chain wear? Who's truck? 7.3L?

That's a 6.4 powerstroke. Timing is gear driven on a majority of modern diesels. This is the result of bad tuning, either written with too much timing or someone put a giant tuner on and just beat the **** out of it. This truck has 2 melted pistons and the heat marks on the rest were pretty close to melting.
